To allow for treatment of a patient who is considered a minor, it is necessary for a parent or legal guardian to give consent for treatment. If a minor child presents for an appointment without a parent or legal guardian or a signed consent, treatment may be denied.
Medication refill requests may take up to 3 business days to process. Please plan ahead and contact our office before running out of your medication. Your provider may require you to schedule a visit before a refill will be processed.
